Homelessness Coordinated Response Team

The Homelessness Coordinated Response Team (HCRT) is a case conferencing group that shares knowledge and resources to support people experiencing homelessness in Northumberland County. Individuals provide signed consent to have their name put on the By Name List, which allows the HCRT Team to discuss information directly related to the individuals' housing situation and goals. 

The current Homelessness Coordinated Response Team meets every two weeks to discuss any situations that have arisen in the community that are presenting challenges for the people they are working with. Partners around the table can share ideas and resources. Specialist help and support can be identified and plans can be put in place to help people experiencing homelessness to access the support and services they need, helping them to remain safe and healthy, and to identify housing options and solutions.
