Colborne Rural Outreach Clinic
The Colborne Rural Outreach Clinic (ROC) is the first clinic of its kind organized by the Ontario Health Team of Northumberland (OHT-N).
Launched April 6, 2021, this clinic is part of OHT-N efforts to bring services to rural community locations in Northumberland. The aim is to reduce barriers to care such as access, transportation and outreach challenges. Colborne ROC services are provided through a collaboration of OHT-N partners and Cramahe Township.
Services are available by appointment only on a first come, first served basis. Walk-in support is not available at the clinic at this time.
Services currently include:
- Primary care with a Nurse Practitioner (for patients who do not currently have a local primary care provider, or those who may need specific temporary support closer to home)
- Adult and youth mental health supports
- Foot care with a Chiropodist
- Diabetes education
- Family violence prevention resources and support
Future services planned for this location include:
- Smoking cessation
- Seniors wellness supports
